What Is the Phlebotomy Certification Exam
The phlebotomy certification exam is a national test that shows an employer you have the knowledge to collect, label and process blood specimens safely. Most employers expect certification even in states that do not require it, and many training programs build the exam into their final weeks.
Several organizations certify phlebotomists, and each sets its own exam. This guide uses the National Healthcareer Association (NHA) Certified Phlebotomy Technician (CPT) exam for the specific numbers, because its test plan, scoring and fees are published and checked for this page. Our free phlebotomy practice test follows the same five content areas.
National Phlebotomy Certification Exams
A national phlebotomy certification is recognized across states rather than tied to one. The content is broadly similar between providers: safety, patient identification, venipuncture and capillary technique, order of draw, special collections and specimen processing. What differs is the eligibility route, question count, scoring method and fee.
NHA, NCCT, ASCP and AMCA Certifications
Phlebotomy certification exams range from 80 questions in 2 hours (ASCP PBT) to 200 questions in 3.5 hours (NAPTP), so check which credential your employer or state expects before you prepare. The figures below come from each certifying body’s own website or candidate guide; a dash means the body does not publish that figure on its exam pages.
| Certifying body | Credential | Questions | Time | Passing score | Fee |
|---|---|---|---|---|---|
| NHA | Certified Phlebotomy Technician (CPT) | 100 scored + 20 pretest | 2 hours | 390 on a 200–500 scale | $134 |
| ASCP Board of Certification | Phlebotomy Technician, PBT(ASCP) | 80 (computer adaptive) | 2 hours | 400 on a 100–999 scale | $155 application fee |
| NCCT | National Certified Phlebotomy Technician (NCPT) | 100 scored + 25 pretest | 3 hours | — | See NCCT fee schedule |
| AMT | Registered Phlebotomy Technician (RPT) | — | 2.5 hours | — | $125, including application, exam and first annual fee |
| NAPTP | Phlebotomy Technician national exam | 200 | 3.5 hours | 75% | $169 (exam only) |
| AMCA | Phlebotomy Technician Certification | — | — | — | See AMCA’s exam outline |
Every one of these exams covers the same core skills: safety, patient identification, venipuncture and capillary technique, order of draw, special collections and processing. Any ASCP, NCCT, AMT or AMCA phlebotomy practice test is therefore useful for the shared content, even though the format differs.
State Phlebotomy Exams
Most states do not license phlebotomists, so a national certification is the main credential. A few states, including California, require state certification or licensure, which usually means completing approved training and passing an approved national exam. If you are looking for a phlebotomy practice test for a state exam, start with your state health department’s requirements and the national exam it accepts.
Phlebotomy Exam Format
The NHA CPT exam has 100 scored questions plus 20 pretest questions that do not count, with 2 hours to complete it. The questions are multiple choice and fall into five content areas under the 2025 test plan, which NHA began using on January 7, 2026:
| Content area | Scored questions |
|---|---|
| Safety and Compliance | 26 |
| Patient Preparation | 20 |
| Routine Blood Collections | 28 |
| Special Collections | 12 |
| Processing | 14 |
Safety and Compliance
Safety and Compliance is the second-largest area. It covers infection control, standard precautions, hand hygiene, sharps safety and needlestick exposure, the OSHA Bloodborne Pathogens Standard, HIPAA, patient consent and the agencies that regulate laboratory work.
Patient Preparation
Patient Preparation tests what happens before the needle: reviewing the requisition, identifying the patient with two identifiers, confirming fasting or other requirements, choosing a site and applying the tourniquet. Sites to avoid, such as an arm with an IV or the side of a mastectomy, appear often.
Routine Blood Collections and Order of Draw
Routine Blood Collections is the largest area. It covers venipuncture and capillary technique, needle gauge and angle, tube additives, tube fill and inversions, and troubleshooting a missed vein. Expect several questions on the order of draw, which prevents additive carryover between tubes:
- Blood cultures and other sterile specimens
- Light blue sodium citrate tubes
- Serum tubes with or without clot activator or gel (red, gold)
- Green heparin tubes
- Lavender EDTA tubes
- Gray sodium fluoride and potassium oxalate tubes
Capillary collections follow a different order, with EDTA collected first, and that difference is a classic exam question.
Special Collections and Processing
Special Collections covers blood cultures, glucose tolerance tests, timed and therapeutic drug monitoring specimens, chain of custody and specimens that must be chilled, kept warm or protected from light. Processing covers labeling, centrifuging, aliquoting, transport and the reasons a lab rejects a specimen, such as hemolysis or an unlabeled tube. These phlebotomy practice test questions reward careful memorization.
Phlebotomy Exam Scoring and Passing Scores
The NHA CPT exam is scored on a scale of 200 to 500, and you need a scaled score of 390 to pass. Scaled scoring adjusts for small differences in difficulty between exam versions, so NHA does not publish a fixed number of questions you can get wrong.
The 20 pretest questions are not scored, but you cannot tell which ones they are, so answer every question carefully. Other certifying bodies use their own scales and passing standards, which are listed in their handbooks.
Registration and Fees by Certification
The NHA CPT exam fee is $134 per attempt. You apply through the NHA online store, confirm your eligibility through a training program or qualifying work experience, and then schedule your exam. Many training programs register students directly and include the fee in tuition.
Other bodies set their own fees: ASCP charges a $155 application fee for the PBT, AMT charges $125 for the RPT including the application, exam and first annual fee, and NAPTP lists $169 for the exam only. NCCT and AMCA fees and eligibility routes change from time to time, so use the official handbook for current figures. Ask your training program which certification local employers prefer before you pay for any exam.
Phlebotomy Exam Pass Rates
NHA reports a 75.96% pass rate for its Certified Phlebotomy Technician exam, covering all CPT exam administrations in calendar year 2024. This figure includes retakes, so it is not a first-attempt pass rate.
The other certifying bodies do not publish a common pass rate, so there is no single national figure for every phlebotomy exam. Roughly one in four NHA CPT attempts did not pass in 2024, which is why steady practice before test day pays off.
How to Prepare With a Free Phlebotomy Practice Test
The most efficient preparation combines your training manual with a free phlebotomy practice test taken every few days. A three-week plan works well right after your program ends:
| Week | Focus |
|---|---|
| 1 | Baseline practice test, then safety, compliance and patient identification |
| 2 | Order of draw, tube additives, venipuncture and capillary technique |
| 3 | Special collections, processing, two timed practice tests and review |
Set up the weeks with the study planner and track your exam date with the exam countdown. Tube colors, additives and order of draw are perfect for flashcards, so make flashcards online and review them daily.
Phlebotomy Practice Test With Answers
A phlebotomy practice test with answers is only useful if you study the explanations. Our review shows the correct answer, an explanation and a “why” note for every question, so you learn the rule rather than the answer letter.
Free Phlebotomy Practice Test for Certification
Our free phlebotomy practice test for certification draws 30 random questions from an original bank organized by the five NHA content areas. Your results include a section breakdown, so you can see whether processing or special collections needs another review.
Test Day Checklist
Arrive prepared, calm and early. Check the rules in your certifying body’s candidate handbook, and use this list as a starting point:
- Bring the valid government-issued photo ID your certifying body requires, with a name that matches your registration.
- Confirm your exam time, location or online proctoring requirements the day before.
- Arrive early or log in early if you test remotely.
- Leave phones, notes and smartwatches outside the testing area.
- Read every question fully, especially on order of draw and special handling.
- Answer every question, because unanswered questions cannot earn points.
Phlebotomy is often one step in a wider health care career. If you are also considering patient care roles, compare the CNA exam and the CNA practice test, or read our guide to the EMT exam.
Phlebotomy Exam FAQ
Is the Phlebotomy Exam Hard?
The phlebotomy exam is very passable for candidates who finish a training program and study the details. The questions are practical, but they test precise rules such as the order of draw, tube additives, tourniquet time and patient identification. Most people who struggle miss safety, special collection and processing questions rather than basic venipuncture.
What Is the Passing Score for the Phlebotomy Exam?
For the NHA Certified Phlebotomy Technician (CPT) exam, the passing score is 390 on a scaled range of 200 to 500, according to the NHA candidate handbook. Other certifying bodies such as NCCT, ASCP and AMCA set their own passing standards, so check the handbook for the credential you are taking.
Is the Phlebotomy National Exam Hard?
National phlebotomy exams are moderately challenging. NHA reports a 75.96% pass rate for all CPT exam administrations in 2024, which means roughly three in four attempts passed. Candidates who practise with timed questions and know the order of draw thoroughly are usually well prepared.
How Many Questions Are on the Phlebotomy Certification Exam?
It depends on the certifying body. The NHA CPT exam has 100 scored questions plus 20 unscored pretest questions in 2 hours, under the 2025 test plan used from January 7, 2026. ASCP's PBT has 80 questions in 2 hours, NCCT's NCPT has 100 scored and 25 pretest items in 3 hours, and the NAPTP exam has 200 questions in 3.5 hours.
