How Our Free NCLEX-RN Practice Test Works

Our free NCLEX-RN practice test draws questions at random from an original bank written to the NCSBN client need categories. Each attempt mixes management of care, safety, health promotion, psychosocial and physiological questions, so no two sessions are identical.

Choose a mode before you start. Timed mode gives you 60 minutes for 30 questions, untimed mode removes the clock and study mode shows the answer and rationale after each question. You can flag questions, move around with the question navigator and answer with the number keys.

When you submit, you see your score, percentage, time used and a breakdown by client need category. For the full NCLEX-RN exam format, scoring rules and registration steps, read our NCLEX-RN guide.

NCLEX Practice Questions With Answers

Every item comes as one of our NCLEX practice questions with answers you can review in full. The answer review shows your choice, the correct answer, an explanation and a short “why” note on the principle behind it. Some items are select-all-that-apply. Our version tells you how many options are correct, which the real exam does not, and you must choose every correct option to earn the point.

NCLEX Practice Questions Online

These NCLEX practice questions online run in any modern browser with no account or download. If you close the tab mid-test, your progress is saved on your device so you can resume, and your last ten results are kept so you can watch your scores improve.

What the Real NCLEX-RN Practice Test Covers

The real NCLEX-RN covers four client needs split into eight categories: Management of Care, Safety and Infection Prevention and Control, Health Promotion and Maintenance, Psychosocial Integrity, Basic Care and Comfort, Pharmacological and Parenteral Therapies, Reduction of Risk Potential and Physiological Adaptation. Our bank uses the same eight categories, and the sections below show the topics you will meet most often.

Safe and Effective Care: Infection Control

Safe and Effective Care Environment combines management of care with safety and infection control. NCLEX practice questions on infection control ask which isolation precautions a client needs, how to remove protective equipment and what to do after a needlestick. Management of care questions test delegation, assignment, consent and confidentiality.

Health Promotion: Growth and Development

These questions test developmental milestones, screening and family teaching across the lifespan. You will also see NCLEX practice questions on maternity, labor and delivery and pediatrics, such as calculating a due date, assessing the postpartum fundus or choosing safe toys for a toddler.

Psychosocial Integrity: Mental Health

Psychosocial items ask for the most therapeutic response, the safest action for a client at risk of suicide or the right approach to hallucinations, grief or substance withdrawal. Read every option closely, because the wrong answers often sound kind but block communication.

Physiological Integrity: Pharmacology

Pharmacology questions test high-alert drugs such as heparin, warfarin, insulin, digoxin, lithium and potassium chloride. NCLEX practice questions on pharmacology usually ask what to check before giving a drug, which side effect to report or what to teach the client.

Physiological Adaptation: Fluid and Electrolytes

Physiological Adaptation covers acute and chronic conditions. NCLEX practice questions on fluid and electrolytes ask you to link a lab value, such as high potassium or low calcium, to the finding or action that matters most. Shock, diabetic ketoacidosis and increased intracranial pressure appear here too.

Med Surg NCLEX Practice Questions

Med surg NCLEX practice questions span Reduction of Risk Potential and Physiological Adaptation: chest tubes, postoperative care, transfusion reactions, heart failure and NCLEX practice questions for cardiovascular system and respiratory system disorders. Expect questions about which client to assess first.

Next Generation NCLEX Practice Questions

The real exam includes Next Generation NCLEX items such as matrix, bowtie and trend questions, often inside case studies. Our bank does not reproduce those layouts. Instead, its clinical judgment items train the same steps: recognizing cues, prioritizing problems, choosing actions and evaluating outcomes.

Scoring Explained for Free RN NCLEX Practice Questions

The real NCLEX-RN has no percentage pass mark. It is pass or fail against a passing standard of 0.00 logits set by NCSBN, and the adaptive exam stops once it is 95% certain of your result. Our practice test therefore uses a 70% readiness benchmark, clearly labelled on the results screen.

Scoring above 70% on several different random sets is a good sign that you are ready. Scoring below it tells you which categories to review before your next attempt.

NCLEX Practice Questions RN Results by Category

The category breakdown is the most useful part of your results. A strong total can hide a weak area, such as pharmacology or management of care, that the adaptive exam will find quickly. Review the weakest category first, then retake a mixed set to check the improvement.

Tips to Pass the NCLEX-RN

The candidates who pass the NCLEX-RN usually practise every day, organize their review by client need and treat every rationale as a mini lesson. Studying NCLEX practice questions by topic keeps your review focused.

How Many NCLEX Practice Questions Should I Do

There is no official number, but many candidates answer a few thousand questions over six to eight weeks. Quality beats volume: 75 questions reviewed properly teach more than 200 rushed ones. Plan daily sets with the study planner and track your date with the exam countdown.

Review Every NCLEX Practice Question With Rationale

After each set, read the rationale for every NCLEX practice question, right or wrong. Write the rule behind each miss on a card, and make flashcards online for lab values, drug classes and isolation precautions you keep forgetting.

Practice Prioritization Questions

Prioritization questions ask which client to see first or which action comes first. Use airway, breathing and circulation, then safety, then unstable over stable clients. General test taking strategies, such as ruling out answers that delay care, help on these items.

Free NCLEX Practice Questions

These free NCLEX practice questions cost nothing, need no sign-up and can be repeated as often as you like. Each attempt draws a fresh random set, so you will not simply memorize an order.

NCLEX Practice Questions for Free

Using NCLEX practice questions for free is a smart complement to a paid review course. Use them for daily warm-ups, quick checks on a weak category or a final review the week before your exam.

NCLEX-RN Practice Test FAQ

What Kind of Questions Are on NCLEX?

The NCLEX uses multiple-choice, select-all-that-apply and Next Generation NCLEX item types such as matrix, drop-down cloze, drag and drop, highlight, bowtie and trend items. Many sit inside case studies about one client. Our practice test focuses on single-answer and select-all-that-apply questions that build the same clinical judgment.

How Many Questions Are on RN NCLEX?

The RN NCLEX gives between 85 and 150 items within a five-hour limit, according to the NCSBN test plan effective April 2026. Because the exam is adaptive, it can stop at any point in that range once the result is clear. Our practice test uses 30-question sets, with options for 10 questions or the full bank.

Is the NCLEX-RN Exam Hard?

It is challenging because the adaptive design keeps serving questions near the limit of your ability, so nearly every candidate leaves unsure. The questions reward prioritization and safety rather than memorized facts. Regular practice with rationales, organized by client need category, is the most reliable way to prepare.

What Is NCLEX-RN Practice Test?

An NCLEX-RN practice test is a set of exam-style questions you answer to rehearse the licensure exam. This one is free, draws questions at random from an original bank covering all eight client need categories and explains every answer, so you learn the reasoning rather than memorizing a list.

Is NCLEX-PN Harder Than NCLEX-RN?

The two exams are equally rigorous for their own scope of practice. The NCLEX-RN expects more management of care, delegation and complex judgment, while the NCLEX-PN focuses on coordinated care in the practical nurse role. Both share the same adaptive format, the same 85 to 150 item range and the same five-hour limit.
